Add Fields to the Donation Import Template
Add Middle Name, Suffix, Salutation, Mailing Label, and Recognition Name fields to the donation import template. This will alleviate some of the manual work needed on profiles after a donation import.

Add "Salutation" and "Suffix" fields to the import donations template so that we don't have to manually add these to the profiles that get created by the import.
