Print Packets - Form Title on Cover Page
It would be very helpful to have the form title on the cover page of the print packets. We compile copies of the application print packet, evaluation 1 print packet and evaluation 2 print packet and it would be very helpful to see which form it is on the first page of the packet.

This is something we would like to see too! It is so confusing to not see the form name on the front page of the printed packet, especially when you're looking at multiple reports.

Yes please! My board likes printed copies and are often confused about what they are reading. We ask for reports (follow ups) every six months from our grantees, but some will submit interim reports, some annual reports (for multi-year grants), and some final reports. What the board sees on a pdf print package is the initial application title, so they have no idea what they're reading unless I write it on the front page. Thank you!

I agree. Since we are naming our form, it would be appropriate to have the title of the form appear at the top of the form.
