Include donor and fund name on the new tribute notify report
We don't always use templates to send tribute notifications, so it's very useful for us to be able to see the tribute, whom to notify, the fund name, and the donor's name. The old tribute notify list included this, and it'd be really wonderful to have that feature back!!

Fund name, at a minimum, would be helpful as we have multiple supporting organizations and affiliates working within the system. Fund name would allow the leaders of each group to easily identify the notify letters they need to process.
